Membership Based Practice

Primary Care Membership
- All initial visits are $149.
- After initial visit patients can either join the membership or schedule a follow up visit ($149).
- If patient does not want to join the practice after initial or follow up visit, recommendations can be sent to the patient’s primary care provider.
- If patient would like to have Dr. Guminsky as their primary care provider, then they will need to join the membership.

What is included in membership?
- Same day or next day sick visits, unless doctor is out of town.
- Discount on lab work if not using insurance.
- Up to 3 visits a month, includes phone visits (beyond 3, office visits will incur a charge)
- Refills turned around in 48 business hours
